Perry Memorial Hospital Foundation
 
Founded

The foundation was formed in 1990 to assist the hospital in fulfilling it's mission, and is a 501 c.3, tax exempt organization. An endowment or contribution to the foundation is generally tax exempt.

Purpose

To establish goals and objectives that will assist the foundation in providing support to Perry Memorial Hospital. The foundation is also developing fund raising methods that will provide capital resources needed to support the hospital's mission.

Membership
The Foundation Board consists of 7 members who are dedicated to supporting Perry Memorial Hospital's Mission. Foundation members include members of the PMH Trust Authority as well as private individuals.

Since the foundations inception Perry Memorial Hospital has benefited from the community's generous donations:

1991 - $300 for the purchase Wheelchair
1994 - $150,000 for the purchase Assisted in the purchase of a CT Scanner
1996 - $75,000 for the purchase Ultrasound Machine
1997 - $350 for the purchase Phaco Handpiece (Cataract Surgery)
1997 - $15,856.62 for the purchase Cardiac Life Pak Defibrillator
1997 - $1,370 for the purchase PCA II Pump
2001 - $150,000 for the purchase R&F System
2001 - $15,974.32 for the purchase Telemetry System
2002 - $30,747 towards the purchase of 12 Beds, over the bed tables, bedside tables
           and 4 sleeper sofas. These funds came from restricted donations.
2003 - $27,477.86 for the purchase of Anesthesia Machine
2003 – $9,945.60 for the purchase of LIFEPAK 12 Defibrillator
2004 - $15,679.50 for the purchase of 2 LIFEPAK 20 Defibrillators
2005 – $145,000 for the purchase of CT Scanner and Workstation
2007 - $5,400 to Perry Fire Department for purchase of 2 AED’s
2007 - $1,400 for the purchase of 5 Televisions from restricted funds
2008 - $2,600 for the purchase of Ventilator
2008 - $945.45 for the purchase of Blood Pressure Monitor
2008 - $1,367.64 for the purchase of Pulse Oximeter
2008 - $35,768 for the purchase of Bone Density Unit
2008 - $445.35 for the purchase of 2 wheelchairs from restricted funds
2008 - $6,835.32 for the purchase of Linen Cart
2008 - $200 for the purchase of Venoscope II: Transilluminator
2008 - $4,188.57 for the purchase of Ice Machine

The foundation has also awarded $14,200 in scholarships to students continuing their education in the field of health care.

The Perry Memorial Hospital Foundation choose the Tree of Hope as it typifies the nature of the people of the Oklahoma Land Run.  The tree will grow at a rate proportionate to the desire the citizens of Noble County have for helping others.  Just as the land run opened up homestead rites, funds raised by the Tree of Hope will provide hope for the continued growth of progressive and preventive healthcare services in our community.
