Community Health Needs Assessment

The Patient Protection and Affordable Care Act (PPACA), commonly called Obamacare or the Affordable Care Act, is United States Federal Statute signed into law by President Barack Obama on March 23, 2010.  Under the Patient Protection and Affordable Care Act, Perry Memorial Hospital is required to conduct a community health needs assessment, at a minimum, every three years.  In 2013, PMH partnered with Oklahoma Rural Health Works which is a collaborative effort of the Oklahoma Office of Rural Health and Oklahoma Cooperative Extension Service, to facilitate the 2013 Noble County assessment.   The complete Plan can be viewed using the following link:
