Perry Memorial Hospital has trained staff to assist you and your family when life’s difficult decisions involving end-of-life care must be answered.
An advance directive, or living will, allows you to document, in advance, your decisions about end-of-life care. Communicating your wishes in advance helps family, friends and healthcare professionals avoid confusion when emotions are running high.
Using the State of Oklahoma Advanced Planning forms provided below, our staff can help walk you through each question, giving you ample time and support to consider each aspect.
In Oklahoma, there are legal tools for advance planning which set out individual healthcare decisions. They are:
- Advanced Directive for HealthCare; Living Will/Appointment of Health Care Proxy
- Do Not Resuscitate form; and
- Durable Power of Attorney for Health Care.
